1. Please complete the following form to indicate your workshop choice. Then click the SUBMIT button which will send your information to CPSA.
2. On the next page, please click Add to Cart next to the Awards Banquet listing. You will have a chance to "View Cart" before your final Check Out. The payment section takes place on the secure PayPal site.  You will be returned to the CPSA Website when your purchase is completed.
3. Select your preferred dates for workshops for yourself and additional attendees (if you are registering for others.)  Then complete the name and address form below. For details on workshops Prospectus (pdf file)
Please note: There is a $20 charge for each cancelled registration. No refunds after July 23rd. If either workshop has fewer than 30 sign up for both days (combined), it will be held just once on Friday, August 1st. Anyone wishing to switch workshops and/or days MUST obtain permission from Patti Greene (finance@cpsa.org) or Cheryl Cox (conventions@cpsa.org).
